Our staff works closely with hospital discharge planners and home care agencies as they assist you through the health care continuum. Prior to admission, our admissions coordinator will evaluate each potential resident’s level of care requirements in order to accommodate placement needs.
The admission coordinator will also assist residents and their families with completing the required admission application.
Our staff is familiar with the required paperwork for Medicaid, Medicare and private insurance companies. We are happy to assist you in understanding the process of applying for admission, as well as the payment options for placement in a skilled nursing facility.
New Glen Oaks rehabilitation unit specializes in short-term therapy, and our team works with you to build a unique plan of care to meet your needs and goals.
